Five trials opened this week, and between them they cover groups who are usually left out of GLP-1 research, including kidney transplant recipients and people with Type 1 diabetes. None has started dosing. What they change is the question of who gets studied, which eventually decides who gets covered.
A Canadian trial will test whether oral semaglutide can prevent diabetes after a kidney transplant. Diabetes that begins after a transplant is common and it threatens the transplant itself, so prevention here is about protecting an organ rather than about weight.
A large Phase 3 trial is recruiting people who have obesity together with heart failure with preserved or mildly reduced ejection fraction, testing maridebart cafraglutide. That form of heart failure has very few effective treatments, which is why this group keeps appearing in GLP-1 research.
A Phase 2 study will test tirzepatide in people with Type 1 diabetes. These medications are approved for Type 2, and people with Type 1 who also carry extra weight have had almost no evidence to point their doctors at, so this one answers a question our own members ask often.
The remaining two are earlier stage and are about what comes next. Novo Nordisk is recruiting 230 people with obesity for a Phase 2 trial of an experimental drug, and Alnylam is testing a new compound both on its own and in combination with tirzepatide.
Nothing here is prescribable, and the two experimental drugs are years from any decision. The useful part is which groups are being studied at all. Coverage follows evidence, and for kidney transplant recipients, people with heart failure, people with Type 1 diabetes and the people waiting on the next drug, that evidence is only now being gathered.
