Medicaid data from the Centers for Medicare and Medicaid Services, covering the first quarter of 2026, shows GLP-1 spending moved in different directions depending on the state. Some states paid out much less than they did the previous quarter, while others paid out noticeably more. These are Medicaid spending and prescription figures, and the amounts are what Medicaid reimbursed pharmacies before manufacturer rebates. Here is what the numbers show, state by state and nationwide.

The swing in California is hard to miss. California's Medicaid drug utilization data for the first quarter of 2026 is dated March 31, 2026. Prescriptions for Wegovy, Zepbound and Saxenda fell from 523,015 in Q4 2025 to 126,912 in Q1 2026. That is a steep drop in just one quarter. California's total Medicaid GLP-1 spending fell from $989.6 million in Q4 2025 to $537.6 million in Q1 2026, and its total GLP-1 prescriptions fell from 882,881 to 498,483.

Not every state moved the same direction. North Carolina's Medicaid program reported GLP-1 spending of $145.7 million for Q1 2026, dated March 31, 2026. That is up from $89.6 million in Q4 2025. North Carolina was named one of the five highest-spending states for GLP-1 medicines in Medicaid in Q1 2026.

Missouri followed a similar path. Missouri's Medicaid program reported GLP-1 spending of $121.2 million for Q1 2026, dated March 31, 2026, up from $102.2 million in Q4 2025. Missouri was also named one of the five highest-spending states for GLP-1 medicines in Medicaid in Q1 2026.

Put the states together and the national trend looks different from some of the individual stories. Medicaid paid $2.58 billion nationwide for 2,412,872 GLP-1 prescriptions in Q1 2026, dated March 31, 2026. That is a 20% drop in dollars from the $3.22 billion Medicaid paid for 3,012,864 prescriptions in Q4 2025. The overall direction, in other words, was down.

There is also a separate Medicare item. Trulicity is on the CMS selected drug list for Medicare price negotiation, and CMS's most recent update to its entries is dated January 27, 2026. Any negotiated price would take effect on January 1, 2028, and CMS has not yet posted a negotiated price. The negotiated price applies to Medicare Part D and is not a patient copay.

nltimes.nl reports that Novo Nordisk warned the stolen pens could be sold online as weight loss medication, but they contain insulin rather than semaglutide, the active ingredient in weight-loss drugs. The New York Post reports that FDA inspectors found mold and bug infestations at two Chinese factories making GLP-1 drug components. Fox News reports that a Maryland man named Engel has sued Novo Nordisk, alleging the company failed to warn that Ozempic could cause vision loss.

That climb helped push Lilly onto a list of stocks IBD flagged as top performers this week, with the outlet's headline crediting Lilly as leading 19 stocks to best stock lists. Those FDA findings come from an inspection that the New York Post describes as showing gross contamination at two factories in China that make components for GLP-1 weight-loss drugs. According to the Post, citing Bloomberg, both facilities are run by compounding pharmacies that produce semaglutide and tirzepatide for black-market versions of the drugs not approved for sale by safety regulators. The Post reports that both companies were placed on the FDA's import list, meaning their products could be confiscated at the border. A separate report cited by the Post found that staff at Nanjing Chengong Pharmaceutical had been buying active ingredients from other factories and applying their own company's seal of approval without reviewing testing and safety documentation. Fox News reports that a Maryland man named Engel has filed a lawsuit against Novo Nordisk, alleging the company failed to adequately warn patients and doctors about the potential risk of vision loss from Ozempic. According to Fox News, Engel says the drug caused total vision loss, leaving him legally blind. The outlet reports that Engel is among hundreds of patients currently pursuing legal action against makers of GLP-1 medications, including Ozempic and Wegovy, over claims that the drugs caused NAION. Fox News notes that Novo Nordisk disputes that the medication causes NAION, stating that the available evidence does not establish a causal relationship between semaglutide and the eye condition. And overseas, growth forecasts are in focus.

financialexpress.com reports that Novo Nordisk forecasts 15 to 20 percent annual growth for GLP-1 drugs in India amid rising competition from cheaper versions. The outlet says competition intensified after semaglutide's patent expired in March, allowing domestic drugmakers to introduce lower-cost alternatives. According to financialexpress.com, Novo Nordisk has submitted applications for higher-dose semaglutide formulations, including a 2 mg dose for diabetes and a 7.2 mg dose for obesity, both under regulatory evaluation. The outlet also reports that Novo's oral semaglutide pill, approved specifically for obesity in the US, became available there in January 2026.

Ireland's health service is weighing whether to pay for more GLP-1 medications, according to the Irish Times. In South Africa, regulators are revising the rules on how pharmacies customise these medicines.

The Irish Times reports that the HSE is considering reimbursing more GLP-1 medications after receiving a number of applications. The outlet notes that in December 2025, the National Centre for Pharmacoeconomics said Mounjaro was not cost-effective but that the HSE should consider reimbursing it if it could agree a price reduction with manufacturer Eli Lilly. According to the Irish Times, more than 45,000 people are getting Ozempic for free under the Long-Term Illness Scheme, with more than 8,000 receiving it under the medical card, and Saxenda remains the only GLP-1 therapy currently reimbursed for weight loss by the HSE.

In South Africa, the focus turns to how these medicines get made. Sundayworld.co.za reports that the country is revising its rules governing the customisation of medicines, including GLP-1 treatments for diabetes and weight loss, with health regulators warning pharmacies not to act as small-scale manufacturers. The outlet reports that the revised standards are expected to go before the council in February, with publication targeted for March. Sundayworld.co.za notes that the popularity of GLP-1 medicines such as Ozempic, Wegovy, Mounjaro and Zepbound has fuelled growth in compounded versions, drawing scrutiny from regulators globally, and reports that a council representative named Tlala said the council was concerned pharmacies were compounding medicines in anticipation of demand rather than for specific patients.

Three items touching GLP-1 research are covered here, from a trial that is not yet recruiting to a study on mental health outcomes. If you take or are considering a GLP-1 medication, these updates show what each item reports and what it does not.

The FutureShape Family trial, sponsored by Signe Torekov, is listed as Phase 4 and is not yet recruiting participants. ClinicalTrials.gov lists a start date of October 1, 2026 and a completion date of January 31, 2031. The trial is registered under NCT07872904 and lists obesity and overweight as its conditions. Interventions listed are family care, standard care, semaglutide (administered by PDS290 pen-injector) and placebo (administered by PDS290 pen-injector). Enrollment is listed as 400. So there is nothing to report yet beyond the registration itself.

A preprint posted on medRxiv looked at whether drugs proposed to protect or harm joints change serious osteoarthritis events, drawing on adverse event tables from 881 randomized trials. It found that GLP-1 receptor agonists and SGLT2 inhibitors had no detectable effect on osteoarthritis-related serious adverse events over about two years, in trials that detected their known effects. The preprint says slow or modest structural effects remain possible, and the paper has not been peer reviewed.

An observational cohort study in The Lancet Psychiatry, using electronic health records data from the USA, followed adults with schizophrenia and type 2 diabetes. It found that starting GLP-1 therapy was associated with lower psychiatric acute-care use than not starting it or starting a DPP-4 or SGLT2 inhibitor. The study notes that potential residual confounding and selection bias restrict how broadly the results can be applied.

These three items are different kinds of evidence: a trial registration with no results, a preprint that has not been peer reviewed, and an observational study that shows an association rather than a cause.

A Phase 4 trial recruiting at Universitaire Ziekenhuizen KU Leuven is trying to answer a specific question about semaglutide and eating behavior. The trial, A Neuropsychobiological Approach to Optimize Patient Selection for GLP-1-based Pharmacotherapy for Weight Management Across the Binge Eating Spectrum, is sponsored by Universitaire Ziekenhuizen KU Leuven and is listed under identifier NCT07870538. Amgen's Phase 3 trial on at-home use of maridebart cafraglutide has not yet opened for recruitment. The study, Home Self-administration of Maridebart Cafraglutide With Autoinjector in Adults Living With Obesity or Overweight, is sponsored by Amgen and registered as NCT07871409, with a status of Not yet recruiting as of October 13, 2026. Its main objective is to assess whether participants can self-administer subcutaneous doses of maridebart cafraglutide at home using an autoinjector device, with a planned enrollment of 230 people. Sheba Medical Center's Phase 2 ELITE trial on healthy aging has finished running, though its results are pending. ELITE, short for Evidence-Based Longevity Interventions and Targeted Engagement, was a randomized, open-label, two-active-arm pilot conducted at Sheba Medical Center in Ramat Gan, Israel, with a status of Completed (results pending) as of October 13, 2026. The study aimed to characterize individual response to multidomain healthy-aging interventions over about eight weeks, combining a clinical measure of muscular, immune, and cognitive function with frequent digital monitoring between visits. A preprint posted to medRxiv looked at what happens when nutrition therapy is added to incretin-based obesity care. The preprint, Real-World Twelve-Month Weight Loss Outcomes in a Comprehensive Obesity Care Program Utilizing Individualized Nutrition Therapy and Incretin-Based Obesity Medications, carries the identifier 10.64898/2026.10.09.26365158 and is dated October 13, 2026, with a status of preprint, not peer reviewed. It found that integrating individualized carbohydrate-reduced nutritional therapy with incretin-based obesity medication care was associated with greater 12-month weight loss than incretin-based care without structured nutritional therapy. Keep in mind that this has not yet been peer reviewed.

STAT News reports that a new trial, announced on Oct. 13, 2026, will enroll up to 1,200 people with alcohol use disorder, opioid use disorder, or both, starting early 2027. Change is coming to another product too. MedPage Today reported on Oct. 13 that Novo Nordisk announced plans to discontinue its liraglutide injectable Saxenda in early 2027 due to declining sales. Fool.com reports a drug combination with tirzepatide produced more weight loss than tirzepatide alone in a trial, though discontinuation was also higher.

STAT News reports that a new trial, announced on Oct. 13, 2026, will test the real-world impacts of weekly semaglutide injections, focusing on factors like a person's involvement with the criminal justice system, employment, housing stability, and health care utilization. The study, set to begin in early 2027, will enroll up to 1,200 participants with alcohol use disorder, opioid use disorder, or both, according to STAT News. STAT News also reports that a wave of clinical trials have begun to test the ability of GLP-1 weight loss drugs to quell various substance use disorders, with some promising results.

Attention also turned to a different kind of connection inside one company's own data. Yahoo Finance reported on Oct. 13, 2026 that Eli Lilly's TOGETHER PsO data pairing Taltz with Zepbound highlight a potential immunometabolic link between psoriasis and obesity. According to Yahoo Finance, the data strengthens the idea that cardiometabolic and immunology assets can work together, though it does not materially change the near-term focus on obesity and diabetes volumes or the key risk around GLP-1 pricing pressure and litigation exposure. Yahoo Finance also reports that one narrative about Lilly projects $118.9 billion in revenue and $50.1 billion in earnings by 2029, with some analysts assuming figures closer to $139 billion in revenue and $56 billion in earnings.

There's a product update worth noting as well. MedPage Today reported on Oct. 13, 2026 that Novo Nordisk announced plans to discontinue its liraglutide injectable Saxenda in early 2027 due to declining sales.

A guideline announcement also appeared in the same MedPage Today report from Oct. 13, 2026. MedPage Today reports that the World Health Organization announced its first obesity guidelines for children and adolescents, recommending against using weight-loss drugs or bariatric surgery in patients younger than 10 years of age.

Trial results reported by Fool.com on Oct. 12, 2026 point to a tradeoff between effect and tolerability. Fool.com reports that patients taking tirzepatide together with eloralintide, an experimental drug that targets the hormone amylin but not GLP-1, lost up to 23.3% of their weight after 48 weeks, compared with 14.8% for those on tirzepatide alone. The same report notes the discontinuation rate was as high as 27%, which Fool.com suggests may point to tolerability concerns.

Walmart confirmed to CNBC on Tuesday, October 13, that it plans to cover more than 250 prescriptions at no cost to its employees in 2027, including Ozempic and Mounjaro for people with Type 2 diabetes after a new prior authorization. CBS News reports the FDA has received reports that two people died after taking bootleg versions of the experimental drug retatrutide, though the details are unverified. And ABC 17 News reports that GLP-1 medications appear to slow the body's aging processes, a sign the drugs could ultimately help people live longer.

Walmart, the biggest private employer in the U.S., will offer Novo's diabetes injection Ozempic and Eli Lilly's rival shot Mounjaro without charge for people with Type 2 diabetes after a new prior authorization, CNBC reports. CNBC reports Walmart plans to cover more than 250 prescriptions, including some insulin, inhalers and GLP-1s for diabetes, at no cost to its employees in 2027. The company also said in a statement that it is minimizing premium increases for employees, raising rates for its starting plan by $1.50 per pay period to $39.80.

MedPage Today reports that over 24 months of follow-up, there was no sustained or progressive change in anti-VEGF treatment after patients started a GLP-1 agonist, compared with those who did not. The study retrospectively analyzed data for two propensity-matched groups of 9,896 patients with DME, according to MedPage Today. MedPage Today also reports that AION occurred at an average age of 62.98 among GLP-1 agonist users versus 66.45 among non-users. Among second-eye events, 37.5% occurred within 3 months in GLP-1 agonist users versus 22.5% in non-users, and 56.8% versus 40.6% within 6 months, the outlet reports.

Fool.com reports that Eli Lilly announced in July that retatrutide achieved average weight loss of 22.6% over 80 weeks in a Phase 3 trial involving patients with obesity and established cardiovascular disease. According to Fool.com, Lilly plans to submit retatrutide for FDA approval in the first quarter of 2027. The outlet notes Lilly will report third-quarter earnings on Oct. 29, with the market paying particularly close attention to sales of Mounjaro and Zepbound.

ABC 17 News, carrying a CNN report, says GLP-1 medications for weight loss and diabetes appear to slow the body's aging processes, a sign the drugs could ultimately help people live longer. The outlet reports that studies have found GLP-1 drugs improve the health of many different parts of the body, lowering the risk of heart, kidney and liver disease and reducing sleep apnea and inflammation. ABC 17 News also reports that Duffin presented new data from Lilly's Surmount V trial, which randomly assigned more than 700 adults who were overweight but did not have diabetes to use either tirzepatide or semaglutide for 72 weeks, and that across different types of epigenetic clocks researchers saw an average change of between 1.1 and 0.7 years in biological aging. The outlet reports that by measuring changes to proteins in people using semaglutide, the company's scientists found it reduced the heart's biological age by two to four years compared with a placebo, and reduced the age of organs such as the lung, kidney, brain and pancreas by one to four years.

CBS News reports the FDA received reports in recent months that two people died after taking bootleg versions of an experimental weight-loss drug. According to CBS News, the first death reportedly occurred in July, when a healthcare professional told the FDA that a 41-year-old woman with obesity went into cardiac arrest. CBS News also reports that in September, the FDA received a report that a 49-year-old patient died after a licensed nurse convinced her to buy retatrutide from an online peptide vendor, and that she experienced severe dehydration, malnutrition and cardiac arrest. The outlet notes the cases appear to be the first public accounts in the U.S. alleging that unauthorized use of retatrutide, or products claiming to contain it, played a role in a patient's death, though the details aren't verified and it is unclear whether the drug was to blame.

Novo Nordisk has registered a phase 1 trial that would test switching patients from maintenance tirzepatide to a drug called zenagamtide, though it has not started recruiting yet. The other items below are also dated October 6 to 9, 2026, and include a court case and a New York bill about insurer, pharmacy benefit manager and pharmacy control whose description does not mention GLP-1 drugs.

Novo Nordisk A/S is sponsoring the study, which is phase 1 and was last updated October 9, 2026. It has not started recruiting yet. The trial would test which zenagamtide dose people can switch to from a maintenance dose of tirzepatide, with a planned enrollment of 60 adults with overweight or obesity, with or without type 2 diabetes. All participants will get tirzepatide and zenagamtide.

A preprint posted October 6, 2026 on medRxiv looked at what happens after people start these drugs in routine practice. It found that most patients who began semaglutide or tirzepatide did not remain on therapy, reach a maintenance dose, and have weight documented through the first year. The preprint has not been peer reviewed. Documented weight loss was greatest among patients who completed each step of care, and the preprint suggests continuation, dose escalation and monitoring as targets for improvement.

A multicenter phase III trial, published October 7, 2026 in Diabetes, Obesity and Metabolism, tested HD1916, a chemically synthesized semaglutide injection, against the reference product semaglutide sold as Wegovy. The published summary says HD1916 produced equivalent body weight reductions to Wegovy, with a comparable safety profile.

On the legal side, Wampler v. Novo Nordisk Inc is a personal injury product liability case. It is pending in the Eastern District of Pennsylvania, and its latest docket action is dated October 8, 2026.

In New York, Assembly bill A 9184 has 9 sponsors and was reprinted as 9184A on October 9, 2026. It has had no votes. The bill would limit overlapping control between insurance companies, pharmacy benefit managers and pharmacies, and it would require divestment of interests in one or more insurance companies and pharmacy benefit managers. It would also bar insurers, pharmacies and PBMs found in violation from receiving payment from certain state programs. It is still a bill, not a law, and its description does not mention GLP-1 drugs.

The trial registration, the lawsuit and the bill are all ongoing. The HD1916 trial and the preprint have already reported results, and the preprint has not been peer reviewed.

Research presented by Eli Lilly and Novo Nordisk reported that GLP-1 medications reversed biological aging by two to three years in overweight or diabetic patients, according to proteomic aging clocks. The New York Post reported on October 11, 2026 that the difference adds evidence the drugs may also be a longevity treatment.

Novo Nordisk conducted a study that measured blood from 10,052 people, half taking semaglutide and half on placebo, using proteomic clocks to assess biological aging. The New York Post also reported that GLP-1 drugs slowed aging by as much as four years in tests measuring heart proteins, though the effect varied depending on which type of aging clock was used.

The US agency ARPA-H is funding a $38 million study to determine whether semaglutide affects cognition, mobility and sensory acuity in healthy people over 60. That study is a question being asked, not a result. Separately, Fortune published a piece on October 10, 2026 about the Gila monster venom peptide that became the basis for the first GLP-1 medicine, which we covered in our previous edition. Fortune noted that the GLP-1 story shows how patents can reward discovery and push rivals to develop better drugs.

The aging research is still early and the results differed depending on the type of clock used.

A lawsuit between Eli Lilly and Company and Willow Health Services, Inc. is pending in the Ninth Circuit Court of Appeals. Eli Lilly makes GLP-1 medications, but the docket does not say what the dispute is actually about, so there is little we can tell you beyond the fact that it exists.

The case, captioned Eli Lilly and Company v. Willow Health Services, Inc., is pending in the Ninth Circuit. Its most recent docket action is dated March 5, 2026. The docket does not describe the subject matter of the dispute, so we cannot say whether it touches on pricing, supply, marketing, patents, or something unrelated to GLP-1 medications. A case being pending simply means it is active and unresolved. It is not a ruling, a settlement, or any kind of finding against either party.

At this stage, the most accurate thing we can tell you is that this case exists and remains unresolved. We would caution against reading anything further into it until more details become available, whether that means the underlying claims, the parties' arguments, or any outcome from the court.

NPR reported on October 10, 2026 that there is scant data about serious adverse effects of GLP-1 drugs, which makes it harder to answer scientific and legal questions about how often the drugs might cause serious harm like blindness or pancreatitis. A few other recent pieces touch on patents, aging research, natural GLP-1 production, and side effects that are still being worked out.

NPR's reporting points to a specific problem: scant data about serious adverse effects makes it harder to answer scientific and legal questions. NPR says more than 1,000 reports of optic neuropathy linked to GLP-1s have been submitted to the FDA's adverse events database since 2024, along with 1,928 reports of pancreatitis over the same period. The database relies on mostly voluntary reporting that the agency does not verify, and some doctors do not report suspected adverse events because they cannot confirm the drugs caused the harm rather than preexisting conditions. Lawsuits allege that GLP-1 drug makers failed to warn patients and doctors about serious risks including blindness and pancreatitis. These are allegations, not findings.

A separate piece, published by Fortune on October 10, 2026, uses a desert lizard to explain how patents work in the GLP-1 field. Fortune describes how patents can reward discovery and push rivals to develop better drugs. It notes that a peptide isolated from Gila monster venom in 1990 became the basis for the first GLP-1 medicine approved by the FDA in 2005 to treat type 2 diabetes.

Gizmodo reported on October 9, 2026, in a story covered in an earlier edition, that scientists from Novo Nordisk and Eli Lilly presented data suggesting semaglutide and tirzepatide can slow molecular aging clocks compared to placebo. This is not new. Gizmodo notes that the studies were presented at an anti-aging conference, have not gone through peer review, and were conducted by company scientists rather than independent researchers.

Newsmax published a piece on October 9, 2026, also covered in an earlier edition, saying research suggests certain foods can stimulate the body's natural production of GLP-1, a hormone that helps regulate appetite and blood sugar.

MIT Technology Review published a story on October 9, 2026, covered in an earlier edition, under the headline "We're still figuring out the side effects of GLP-1 weight-loss drugs." It asks whether the drugs could affect hair, nails, and vision. It also notes that the most common side effects are gastrointestinal, including nausea, vomiting, diarrhea, and constipation, and that the drugs have been linked to pancreatitis and gallstone disease.

Data on serious adverse effects, such as blindness and pancreatitis, is scant, and researchers are still working out other side effects, such as possible effects on hair, nails, and vision. Newsmax reported on October 9, 2026 that research suggests certain foods can stimulate the body's natural production of GLP-1, a hormone that helps regulate appetite and blood sugar.

MIT Technology Review published a piece on October 9, 2026 titled "We're still figuring out the side effects of GLP-1 weight-loss drugs," which asked whether the drugs could affect hair, nails, and vision. It noted that the most common side effects are gastrointestinal, including nausea, vomiting, diarrhea, and constipation, and that the drugs have also been linked to pancreatitis and gallstone disease.

The New York Post reported on October 8, 2026 on a study of nail changes, which was already covered in an earlier edition of this newsletter and is not new. One study reported GLP-1 users were nearly 25% more likely to have a nail disorder than people not on the drugs. It also reported hair loss in 50% of GLP-1 users versus 34% of those not on the drugs.
